UWM Health Presentations & On-Site Biometric Screenings

HEALTH PRESENTATIONS: Register to attend any of the health presentations available during the on-site biometric screening event. Once at the calendar, select the desired presentation, and click on the Register to this Event link to complete the registration form.

10:30-11:30 am – Eat Right, Live Well presentation from LifeMatters, UWM’s Employee Assistance Program

12:00-1:00 pm – Demonstration & FREE Samples! Cooking Healthy with UWM Head Chef, Matthew Powers

1:30-2:30 pm – Demonstration! Fit Fuel: Healthy Eating & Exercise with UWM College of Health Sciences

1:00-3:15pm - FREE hearing screenings on a first-come/first-serve basis from UWM Community Audiology Services (screenings take approximately 10 minutes)

HEALTH SURVEY ASSISTANCE:  Stop By! Your health plans will be on site to personally familiarize and assist you with their on-line Health Survey, also a necessary step in the Wellness Wisconsin program to receive your $150 pre-paid gift card.

And if you registered for a March 26 On-Site Biometric Screening, please remember to attend your appointmentInitial registration filled early for this date–if you won’t be able to make it, please “cancel” with Optum or contact Benefits at benefits@uwm.edu so others may attend.

Beginning in 2014, adults (18 or older) enrolled in a Wisconsin state group health insurance plan and who complete the Well Wisconsin Program will receive a $150 VISA gift card from their plan.

HOW TO COMPLETE THE WELL WISCONSIN PROGRAM:

1) Take a Wellness Assessment biometric screening, and
           2) Complete your Wisconsin state group health insurance plan’s online Health Survey.

The Well Wisconsin Wellness Assessment biometric screening will measure your height, weight, Body Mass Index, and blood pressure, and collect a blood sample by finger-stick to measure your cholesterol and glucose levels. A biometric screening helps to determine your current level of health & fitness, as well as potential risk factors you should monitor or act upon. The on-site screening process will take less than 30 minutes to complete.

The State of Wisconsin has contracted OptumHealth to provide all on-site biometric screenings; the information collected is completely private & confidential between you, OptumHealth, and your state group health insurance plan (when you complete their online Health Survey and enter this information).
