April 24, UWM On-Site Biometric Screenings & Health Survey Assistance

Time: 8 a.m. until 4:30 p.m.
Location: Ballroom – West; Student Union, 1st Floor

Beginning in 2014, adults enrolled in a Wisconsin state group health insurance plan (includes employees & their dependents, age 18 or older) who complete the Well Wisconsin program will receive a $150 VISA gift card from their plan.


1) Take a Wellness Assessment biometric screening, and
2) Complete a Health Survey online with your Wisconsin state group health insurance plan

HEALTH SURVEY ASSISTANCE ON SITE! Your health plans have again been invited to personally assist you with completion of their 2) online Health Survey, also a necessary step in the Wellness Wisconsin program to receive your $150 pre-paid gift card. Laptops will be available for your personal use to complete the online Health Survey right after you receive your results from your Biometric Screening.

The Well Wisconsin Wellness Assessment biometric screening will measure your height, weight, Body Mass Index, and blood pressure, and collect a blood sample by finger-stick to measure your cholesterol and glucose levels. A biometric screening helps to determine your current level of health & fitness, as well as potential risk factors you should monitor or act upon. The on-site screening process will take less than 30 minutes to complete. Note: Fasting is recommended, but not required.

The State of Wisconsin has contracted OptumHealth to provide all on-site biometric screenings; the information collected is completely private & confidential between you, OptumHealth, and your state group health insurance plan (when you complete their online Health Survey and enter this information).
